feat: add surgery templates

Often surgeries have common set of procedures. It would be nice if doctors can save them as templates and insert them where it needed

this can be a page in the settings where templates can be configured and with Rich Text editor we can show a popup which contains the title, preview of template which can be used to easily select the template and insert it
