NVIDIA RTX 4060 - nvidia-modeset hang on kernel 6.16.3 (Pop!_OS 22.04 LTS)

• GPU: NVIDIA GeForce RTX 4060 8GB
• RAM: 16GB DDR5
• Display: Internal + External monitor (HDMI)

OS & Kernel

• Pop!_OS 22.04 LTS
• Kernel: 6.16.3-76061603-generic
• Driver: nvidia-driver-570-open (version 570.172.08, CUDA 12.8)

Problem
After a system update and reboot, the system froze during boot.
Console showed repeated messages:

INFO: task nvidia-modeset/:883 blocked for more than 122 seconds.
Tainted: G OE 6.16.3-76061603-generic #2025

Temporary Workaround

• Booted into recovery / previous kernel (6.12.x or 6.15.x).
• Reinstalled NVIDIA driver packages.
• System boots normally, but issue can reoccur on 6.16.3.

Notes

• Issue happens with the nvidia-driver-570-open package.
• Running system76-power graphics reports nvidia mode.
• External monitor black at crash.
