Comment lines to tops of generated files

In particular:

• include a time-stamp for when the file was created
• Support comment lines at tops of file (# character)
• Support -c command line option for user comments that get added to the comment lines
• Derived files copy comments from source file (e.g. raw log file comments duplicated in translated log file)
