There's a couple of moderately well-known instances where Paradox incorporated incorrect historical information due to using Wikipedia as a proxy rather than actual primary and secondary sources.
The 19th-century forger Edward Williams, better known by his bardic name Iolo Morganwg, fabricated a claim that the region of Wales now called Powys was originally called the kingdom of Teyrnllwg. Someone added this (misspelling it "Ternyllwg" in the process) to Wikipedia in 2013, and the error was then copied into the Charlemagne expansion by renaming Powys prior to 808 and filling a new House Ternyllwg with fifteen characters which prior to Charlemagne had been part of House Gwerthrynion and two characters added for the 769 bookmark. The Youtuber Cambrian Chronicles documented the process of tracking this down to the source last year.
Information on the historical rulers of medieval Ireland was added to Wikipedia in 2006 based on the book Irish Kings and High Kings by Francis J. Byrne, but this is a difficult source to read and errors were introduced as a result which made their way into Crusader Kings 2. Most notably, Murchad mac Donnchada is incorrectly made king of Munster from 1064 to 1068; in reality, Toirdhealbhach mac Tadhg took Munster in 1063 and Donnchad went on pilgrimage to Rome where he died in 1064. The Youtuber Maniacal Inc. went through the history following the Battle of Clontarf to explain the error three years ago, and Paradox credited them when adding a game rule nine months ago to Crusader Kings 3 to replace Murchad with Toirdhealbhach; however, the error was never fixed in Crusader Kings 2.
I was going to file issues but I'd like confirmation whether errors of the kind fall within CleanSlate's purview to begin with before I do that.
