Add feature to save and clear responses

Not able to implement evaluation time feature but should be able to implement option to clear all stored responses for a semester and a separate option to clear forms without deleting saved responses (prompt user to download all responses before clearing database)

Add confirmation dialog with 'Save' and 'Discard' options if changes to form are not saved and user tries to leave page
